Dutchess Health 2000 is a community collaborative organized in 1994 to improve the health of the residents of Dutchess County New York. The coalition includes nearly 30 stakeholders representing hospitals, providers, insurers, business, government, education, health organizations, social service agencies, religious groups, foundations, and others. Guided by an ongoing assessment and planning process, unmet health needs are addressed by committees and through liaison with existing groups. Priorities of the coalition include:

Intervention strategies include promoting healthy behaviors, clinical preventive services, clinical intervention, and other collaborative activities that strive for measurable improvements in health status.
